Yet More Paperwork - Sorry

This is both an apology and a plea.  The ASA have just changed their data protection notice and now require all members to sign up to the new policy. So a huge pile of forms arrived at my house this morning with the request that each of our members fill one in! Forms will be available on the desk, so I'm sorry to ask for yet more paperwork, but if you could collect a form for each member, complete and return it as soon as possible I would be very grateful. 

Please note that for swimmers under 18 both the swimmer and their parent must sign the form.  If the forms could be filled in at club rather than taken away that would be a help.
